Acute Lumbar Sprain — When You Throw Out Your Back

What is acute lumbar sprain

Acute lumbar sprain is an injury in which the muscles, ligaments, and fascia around the lumbar spine become overstretched or develop microscopic tears during a sudden movement or while lifting a heavy object. It is commonly described as "throwing out your back," and Korean medicine diagnoses it as a state in which blood stasis (eohyeol, 瘀血) is stagnant in the meridians and qi-blood circulation has been abruptly blocked. The hallmark is sudden, severe pain that makes movement nearly impossible.

How to respond in the first 48 hours

  • Rest: During the first 1–2 days when pain is severe, rest in a comfortable position, but do not extend complete bed rest beyond 48 hours. Lying down for too long actually delays recovery.
  • Cold compress: Apply a cold compress to the injured area at 20-minute intervals to reduce early inflammation and swelling.
  • Posture: Lying on your back with a pillow under the knees reduces lumbar lordosis and eases pain.
  • Movements to avoid: Avoid twisting the back or bending deeply forward at all costs.

Acute-phase Korean medicine treatment

The core therapeutic principle for acute lumbar sprain in Korean medicine is hwalhyeol-geo-eo (活血祛瘀) — invigorating blood and resolving stasis to restore qi-blood circulation.

  • Acupuncture: In the acute phase, needles are inserted at Ashi points (阿是穴) and at Yaoyangguan (腰陽關), Weizhong (委中), and Renzhong (人中). In particular, bloodletting at Weizhong (刺絡) directly drains blood stasis and provides immediate pain relief.
  • Cupping (拔罐): Wet cupping over the painful area draws stagnant blood stasis and metabolic waste to the surface and increases local blood flow.
  • Herbal medicine: Danggui-susan (當歸鬚散) or Boguanghwalhyeol-tang (復元活血湯) are used to clear blood stasis and accelerate recovery of injured tissue.
  • Pharmacopuncture: Placenta (Jahageo, 紫河車) pharmacopuncture or anti-inflammatory pharmacopuncture is injected into the injured area to deliver tissue-regenerating factors.

Recovery course and recurrence prevention

Most acute lumbar sprains allow a return to daily activities within 1–3 weeks with appropriate treatment. However, the recurrence rate is high, so beginning core stabilization exercises after the acute phase is essential. When lifting objects, bend the knees and use the lower body, and avoid sudden twisting motions.
