Hip Care and Fall Prevention in Older Adults

Prevention is the best treatment for hip fractures

A femoral neck fracture (a fracture at the neck of the femur) in older adults is more than a simple broken bone. The 1-year mortality rate after surgery is approximately 20–30%, and many survivors permanently lose the ability to walk independently. Most of these fractures result from falls, so preventing falls means preventing hip fractures.

Risk factors for falling

  • Reduced lower-limb strength: Age-related sarcopenia weakens the lower limbs and impairs postural control and the ability to recover balance.
  • Impaired balance: Aging of the vestibular system, declining proprioception, and reduced vision combine to worsen balance.
  • Decreased bone density: Osteoporosis allows fractures to occur with minor impacts.
  • Medications: Antihypertensives, sleeping pills, and sedatives can cause dizziness and drowsiness.
  • Environment: Slippery floors, dim lighting, thresholds, and loose carpets all add risk.

Lower-body strength and balance training

The most effective way to prevent falls is to build lower-limb strength and balance.

  • Sit-to-stand: Slowly sit down and stand up without using your hands, 10 repetitions × 3 sets per day.
  • Single-leg stance: Holding onto a wall or chair, stand on one leg for 10 seconds and gradually increase the duration.
  • Heel-to-toe walking: Practice walking heel-to-toe along a straight line.
  • Resistance band exercises: Strengthen hip abduction and extension with elastic bands.

Maintaining musculoskeletal health with Korean medicine

In Korean medicine, age-related weakening of muscles and bones is categorized as deficiency of liver and kidney (gansin-heo).

  • Herbal medicine (Gwanjeoldan): A formula that tonifies liver and kidney and strengthens bone and muscle, supporting bone density and strength preservation.
  • Moxibustion: Moxibustion at Guanyuan (CV4) and Mingmen (GV4) reinforces lower-burner (xiajiao) yang qi.
  • Acupuncture: Stimulation of lower-limb acupoints improves circulation and activates muscle function.
  • Bone density management: Combine calcium and vitamin D supplementation, appropriate sun exposure, and weight-bearing exercise.

Improve the environment too

Installing grab bars in the bathroom, anti-slip mats, adequate night lighting, and removing thresholds are equally important. Fall-prevention effects are maximized when improved physical capacity is paired with a safer living environment.
