Why does the yo-yo effect happen

The yo-yo effect — returning to your original weight or gaining even more after weight loss — is the biggest source of frustration in dieting. Under extreme calorie restriction, the body perceives starvation and undergoes adaptive thermogenesis, lowering the basal metabolic rate (BMR) by 10–20%. When you stop dieting and resume normal eating, the reduced metabolism leads to caloric surplus, and fat is rapidly stored.

Spleen deficiency (脾虛) and water-damp stagnation (水濕停滯)

Korean medicine explains the yo-yo effect through the pathology of spleen deficiency (脾虛) leading to water-damp stagnation. Excessive fasting or restriction damages the spleen-stomach (脾胃), weakening the power to transform and transport (運化) food. As a result, water-damp (水濕) stagnates in the body, causing edema, heaviness, and a constitution that easily gains weight. The phrase "I gain weight just from drinking water" describes this state.

Banggihwanggi-tang (防己黃芪湯)

Banggihwanggi-tang is a prescription for obesity with deficient spleen qi and stagnant water-damp. Astragalus tonifies spleen qi to drive water metabolism, stephania tetrandra strongly drains water-damp, and atractylodes, licorice, ginger, and jujube stabilize the spleen-stomach.

  • For severe edema: add poria and alisma to enhance diuretic effect
  • With dyspepsia: add pinellia and citrus peel to transform damp (化濕)
  • With extreme fatigue: add ginseng and Chinese yam to tonify qi and strengthen the spleen (補氣健脾)

Strategy for normalizing metabolism

To prevent the yo-yo effect, restoring basal metabolic rate comes first. Instead of drastic calorie cuts, maintain a daily deficit of 300–500 kcal and consume 1.2–1.5 g of protein per kg of body weight to prevent muscle loss. Strength training increases muscle mass, naturally raising BMR. Combining herbal medicine with diet and exercise stabilizes spleen-stomach function and metabolism over 3–6 months, enabling weight maintenance without the yo-yo effect.
