Stress-Related Erectile Dysfunction — Korean Medicine Treatment for Heart-Kidney Disharmony and Liver Qi Stagnation

The relationship between stress and erectile dysfunction
A significant share of erectile dysfunction (ED) arises from purely psychogenic factors. In men under 40 in particular, work stress, financial pressure, relationship conflict, and performance anxiety are the main causes. When the stress hormone cortisol stays chronically elevated, testosterone is suppressed and the sympathetic nervous system becomes overly activated, blocking blood flow into the corpus cavernosum of the penis.
Heart-kidney disharmony (心腎不交) and liver qi stagnation (肝鬱氣滯)
In Korean medicine, normal sexual function is maintained when the heart (心) and kidney (腎) communicate. The heart governs the mind and emotions, the kidney governs vitality and reproduction, and these two organs must intercommunicate so that sexual arousal can translate into a physical response. When stress causes heart fire (心火) to rise and kidney water (腎水) to descend, heart-kidney disharmony (心腎不交) ensues and erectile difficulty develops.
In addition, when emotional repression and stress cause liver qi (肝氣) to stagnate, qi and blood circulation is blocked. Because the liver meridian (肝經) passes through the genitals, this directly affects erection.
Soyo-san (逍遙散) plus kidney-tonifying herbs — a combined treatment strategy
Treatment hinges on a dual strategy: relieving liver qi stagnation while simultaneously replenishing kidney essence.
- Soyo-san: Bupleurum, dong quai, white peony, atractylodes, poria, licorice, mint, and ginger smooth liver qi (疏泄) and stabilize the emotions.
- Added kidney-tonifying herbs: Epimedium, morinda root, dodder seed, and goji berry are added to Soyo-san to replenish kidney yang and kidney essence.
- For clear heart-kidney disharmony: Hwangryeon Ah-gyo-tang or Gyotae-hwan (coptis + cinnamon bark) drains heart fire and lifts kidney yang to restore communication between heart and kidney.
Restoring emotional stability and confidence
Psychogenic ED often follows a vicious cycle in which one episode of failure fuels anxiety about the next. While herbal medicine and acupuncture restore the physical foundation, open communication with a partner, reducing performance pressure, and seeking psychological counseling when needed are also important. Regular exercise — especially lower-body strength training — supports both testosterone and confidence at the same time. If nighttime and morning erections are normal, the cause is likely psychogenic rather than organic, which generally indicates a favorable outlook.
