Male Infertility and Sperm Health — Replenishing Kidney Essence with Ojayeonjong-hwan

The reality of male infertility

Male factors are involved in 30–50% of cases of couple infertility. Male infertility is diagnosed when any one of the following falls below the reference values on semen analysis: sperm count under 15 million/mL, motility under 40%, or normal morphology under 4%. Studies increasingly report that sperm quality is declining worldwide, with environmental hormones, stress, and sedentary lifestyle pointed to as major contributors.

Kidney essence deficiency (腎精不足) and sperm formation

In Korean medicine, sperm is a direct product of kidney essence (腎精). According to the principle that "the kidney stores essence," sufficient kidney essence supports favorable sperm count, vitality, and morphology, while a deficit leads to overall decline in sperm quality. Overwork, chronic stress, excessive sexual activity, and alcohol all deplete kidney essence.

Ojayeonjong-hwan (五子衍宗丸)

Ojayeonjong-hwan is a flagship formula for male infertility, composed of five "seed (子)" herbs.

  • Goji berry (枸杞子): Tonifies the liver and kidney and replenishes essence and blood.
  • Dodder seed (菟絲子): Tonifies kidney yang and reinforces the foundation for sperm production.
  • Schisandra (五味子): Astringes kidney qi and prevents loss of essence.
  • Raspberry (覆盆子): Warms the kidney and secures essence (固攝).
  • Plantago seed (車前子): Removes damp turbidity and clears the essence pathway.

These five herbs work synergistically to replenish kidney essence and promote sperm production. Because the sperm production cycle is about 72 days, a course of at least three months is recommended before semen analysis values improve.

Lifestyle habits that harm sperm health

Sperm are highly sensitive to heat. Saunas, hot baths, and resting a laptop on the lap raise scrotal temperature and damage sperm. Alcohol can damage sperm DNA, and smoking can reduce sperm motility by up to 30%. Obesity (BMI 30 or higher) increases estrogen and suppresses sperm production. Regular exercise, antioxidant nutrients (vitamins C and E, selenium, zinc), and adequate sleep are the foundation of sperm health.
