Postpartum Weight Management — The 6-Month Golden Window Diet After Childbirth

Causes of postpartum weight retention

During pregnancy, women gain about 12–15 kg on average. About 5–6 kg is lost after childbirth through uterine contraction and the discharge of amniotic fluid and placenta. The remaining 6–9 kg consists of body fat and fluids that do not come off easily without postpartum management. Postpartum weight retention is not simply due to overeating — it results from the combined effect of rapid fluctuations in progesterone and estrogen, sleep deprivation, parenting stress, and reduced activity.

The 6-month postpartum golden window

If you do not return close to your pre-pregnancy weight within 6 months postpartum, postpartum weight retention is more likely to become entrenched. During this window, fat cells are still mobile and basal metabolic rate is recovering, making weight loss relatively easier. However, given postpartum recovery and breastfeeding, excessive restriction or intense exercise is forbidden, and a safe approach is needed.

Herbal prescriptions safe during breastfeeding

While breastfeeding, herbal selection requires special caution. Strong herbs such as ephedra and rhubarb may affect the baby through breast milk and should be avoided.

  • Safe baseline prescription during breastfeeding: Banggihwanggi-tang (astragalus, stephania, atractylodes, licorice) — tonifies spleen qi while removing water-damp to reduce edema and weight.
  • Strengthening spleen-stomach function: A modified Hyangsayukgunja-tang normalizes digestion and absorption, supporting metabolic recovery.
  • For residual postpartum blood stasis: A modified Saenghwa-tang clears uterine blood stasis, improving abdominal circulation and supporting weight loss.

Postpartum exercise and nutrition

Light exercise can begin 4–6 weeks after vaginal delivery and 6–8 weeks after a cesarean section. Build intensity in the order of walking → stretching → core exercises (after checking for diastasis recti) → aerobic exercise. Breastfeeding mothers burn an additional 500 kcal per day, so weight can decrease without extreme calorie restriction. Maintain a balanced diet rich in calcium, iron, and protein, and use nuts and Greek yogurt for snacks.
