Spinal Stenosis — When Walking Makes Your Legs Numb

Understanding spinal stenosis

Spinal stenosis is a condition in which the central nerve canal of the spine narrows due to degenerative changes, compressing the cauda equina or nerve roots. It mainly occurs after age 50, with hypertrophy of the ligamentum flavum, facet joint enlargement, and disc bulging acting in combination. In Korean medicine, this is viewed as a state of meridian obstruction caused by overlapping kidney deficiency (sin-heo) and blood stasis (eo-hyeol).

Intermittent claudication and how to differentiate from disc herniation

  • Neurogenic claudication: After walking a certain distance, both legs become numb and heavy; sitting or bending forward relieves the symptoms. This is the most characteristic sign of spinal stenosis.
  • Difference from disc herniation: A herniated disc worsens when bending forward, whereas spinal stenosis worsens when extending the back. Disc herniations tend to affect one leg, while stenosis often involves both legs.
  • Differentiation from vascular claudication: Claudication caused by peripheral arterial disease does not improve simply by standing still and is associated with weakened dorsalis pedis pulses, allowing differentiation.

Korean medicine treatment approach

Because structural changes have already occurred in spinal stenosis, the key is to optimize the perineural environment within the remaining space.

  • Modified Ojeoksan (五積散): Used as a base formula for patterns where cold-damp stagnates in the lower jiao; Achyranthes and Eucommia are added to promote lower-limb circulation.
  • Chuna manipulation: Flexion-distraction chuna temporarily widens the cross-section of the spinal canal, reducing nerve compression. Sessions 2–3 times per week are standard.
  • Acupuncture: Needling Jiaji (夾脊) points and Huantiao (環跳, GB30), Weizhong (委中, BL40), and Yanglingquan (陽陵泉, GB34) improves nerve conduction in the lower limb.
  • Pharmacopuncture: Hwangnyeonhaedok-tang pharmacopuncture is administered around the lumbar region to suppress local inflammatory mediators.

Care for restoring walking ability

In the early phase of treatment, it is important not to overexert even if walking distance is short. Cycling, with the back slightly flexed, is a good aerobic alternative that allows exercise without pain. As treatment progresses, walking distance gradually increases, and 3–6 months of consistent treatment is most effective.
