In Korean medicine, infantile atopic dermatitis is sometimes called fetal heat (胎熱). The concept is that heat-toxin (熱毒) from the mother during pregnancy is transmitted and manifests on the skin after birth. As the child grows, those with weak spleen-stomach (脾胃) function cannot fully digest and absorb food, so dampness-turbidity (濕濁) accumulates internally and overflows to the skin, causing eczema and itching. This is called spleen-deficiency dampness-steaming (脾虛濕蒸).

Modified Sopungsan (消風散) treatment
Sopungsan is a representative skin formula that simultaneously eliminates wind (風), dampness (濕), and heat (熱). Its base composition includes schizonepeta, siler, cicada slough, anemarrhena, and gypsum, with modifications according to constitution.
- Heavy oozing and weeping: add atractylodes and coix to strengthen dampness-draining (利濕)
- Dry, cracked skin: add angelica, raw rehmannia, and ophiopogon to nourish blood and moisten dryness (養血潤燥)
- Severe itching: add dictamnus root bark and kochia fruit to stop itching (止痒)
Reducing steroids alongside Korean medicine
For children already using topical steroids long term, abrupt cessation can trigger a rebound. A safer strategy is tapering steroid potency and frequency gradually over 4 to 8 weeks while improving the constitution with herbal medicine. Many parents notice less itching and faster skin regeneration starting 2 to 3 weeks after beginning herbal treatment.
Dietary management
Eliminate allergy-triggering foods such as dairy, wheat, and eggs for 2 to 4 weeks, then reintroduce them one at a time to identify culprits. Probiotics and omega-3 fatty acid supplementation help improve the gut immune environment.
